2. Develop and implement individualized care plans based on evidence-based practices for patients with complex wound, ostomy, and continence needs.
3. Collaborate with interdisciplinary team members to ensure comprehensive and coordinated care for patients with complex wound, ostomy, and continence needs.
4. Educate patients and their families on wound, ostomy, and continence care, providing them with the necessary tools and resources to manage their condition at home.
5. Evaluate and document patient outcomes, adjusting care plans as needed to promote optimal healing and management of wounds, ostomies, and continence issues.
