Job Description

We are seeking a highly motivated and skilled Clinical Staff Pharmacist to join our team at Orlando Health. This position will focus on providing exceptional care to patients during their transition from hospital to home.   1. Conduct medication reviews and ensure safe and effective use of medications for patients during their transition from hospital to home.
  2. Collaborate with healthcare team members to develop and implement patient care plans.
  3. Educate patients and caregivers on medication regimens and potential side effects.
  4. Monitor patient response to medications and make recommendations for adjustments as needed.
  5. Maintain accurate and complete patient records, including medication histories and treatment plans.
  6. Stay up-to-date on current pharmacy practices, including new medications and treatment options.
  7. Participate in quality improvement initiatives to enhance patient outcomes.
  8. Work closely with insurance companies to obtain prior authorizations and resolve any medication coverage issues.
  9. Ensure compliance with state and federal regulations, as well as hospital policies and procedures.
  10. Maintain a clean and organized workspace, including proper disposal of medications and adherence to infection control protocols.
  11. Train and mentor pharmacy students and residents as needed.
  12. Provide exceptional customer service to patients, caregivers, and healthcare team members.
  13. Collaborate with other healthcare providers to identify and resolve medication-related problems.
  14. Participate in multidisciplinary rounds and meetings to discuss patient care plans.
  15. Communicate effectively with patients, caregivers, and healthcare team members to promote continuity of care and patient satisfaction.
  16. Maintain confidentiality of patient information in accordance with HIPAA guidelines.
  17. Demonstrate a commitment to professional development by attending conferences, workshops, and continuing education programs.
  18. Adhere to ethical and professional standards of practice.
  19. Assist with medication inventory management and ordering.
  20. Perform other duties as assigned by the Pharmacy Manager or Director of Pharmacy.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School

  • Current Licensure As A Pharmacist In The State Of Florida

  • Minimum Of 2 Years Of Experience In A Hospital Or Ambulatory Care Setting

  • Advanced Training Or Certification In Transitions Of Care, Such As Board Certified Ambulatory Care Pharmacist (Bcacp) Or Certified Geriatric Pharmacist (Cgp)

  • Strong Communication And Collaboration Skills, With The Ability To Work Effectively As Part Of A Multidisciplinary Team
