  1. Provide high-quality, compassionate care to patients throughout their healthcare journey.
  2. Coordinate and manage patient care in collaboration with other healthcare professionals, including physicians, therapists, and social workers.
  3. Utilize critical thinking skills to assess and develop individualized care plans for patients.
  4. Communicate effectively and efficiently with patients, families, and healthcare team members to ensure continuity and coordination of care.
  5. Advocate for patients and their needs, ensuring they receive appropriate and timely care.
  6. Monitor patient progress and make necessary adjustments to care plans to improve outcomes.
  7. Utilize case management principles to promote efficient use of healthcare resources and reduce healthcare costs.
  8. Collaborate with insurance companies and other third-party payers to ensure appropriate reimbursement for services rendered.
  9. Stay up-to-date with current healthcare practices and trends, and integrate new knowledge into patient care.
  10. Maintain accurate and timely documentation of patient care and progress.
  11. Participate in team meetings and communicate effectively with colleagues to ensure seamless coordination of care.
  12. Serve as a resource and mentor to other healthcare professionals, sharing knowledge and expertise to improve patient care.
  13. Adhere to ethical and legal standards of nursing practice and maintain patient confidentiality.
  14. Continuously monitor and evaluate the effectiveness of care plans and make necessary adjustments to improve patient outcomes.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field.

  • Current Rn License In The State Of Florida.

  • Minimum Of 3 Years Of Experience In Case Management Or Care Coordination.

  • Strong Understanding Of Healthcare Regulations And Reimbursement Systems.

  • Excellent Communication, Organizational, And Critical Thinking Skills.
