1. Provide compassionate and skilled nursing care to patients in their homes.
2. Ensure high-quality patient outcomes through personalized and holistic care.
3. Collaborate with interdisciplinary team members to create and implement individualized care plans.
4. Monitor and assess patient progress, and make necessary adjustments to care plans.
5. Administer medications and treatments as prescribed by physicians.
6. Educate patients and their families on disease management, medication management, and other health-related topics.
7. Communicate effectively with patients, families, and healthcare team members.
8. Maintain accurate and up-to-date patient records and documentation.
9. Adhere to all hospital policies, procedures, and protocols.
10. Participate in ongoing education and training to enhance nursing skills and knowledge.
11. Ensure compliance with all regulatory and accreditation standards.
12. Promote a positive and welcoming atmosphere for patients and their families.
13. Assist in the transition of patients from hospital to home care.
14. Handle emergency situations calmly and efficiently.
15. Maintain confidentiality and sensitivity in all patient interactions.
