
Registered Nurse Care Coordinator, Infectious Disease

- Monitor and assess patient progress, collaborating with the healthcare team to develop and implement individualized care plans.
- Provide education and support to patients and their families regarding infectious disease management, prevention, and treatment.
- Serve as a liaison between patients, healthcare providers, and community resources to facilitate continuity of care.
- Maintain accurate and up-to-date patient records, documenting all care provided and any changes in condition.
- Administer medications and treatments as prescribed by healthcare providers.
- Monitor and maintain infection control measures to prevent the spread of infectious diseases within the healthcare facility.
