  1. Provide exceptional and patient-centered care to individuals in their homes, following established protocols and guidelines.
  2. Conduct thorough patient assessments and develop personalized care plans based on their specific needs and condition.
  3. Coordinate care with other members of the healthcare team, including physicians, nurses, and therapists, to ensure continuity of care.
  4. Administer prescribed medications and treatments, monitor their effectiveness, and make necessary adjustments.
  5. Educate patients and their families on disease management, medications, and lifestyle modifications to promote overall health and well-being.
  6. Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and discharge summaries.
  7. Collaborate with community resources and agencies to provide patients with access to necessary support services.
  8. Adhere to infection control and safety protocols to ensure a safe and healthy environment for patients.
  9. Participate in ongoing education and training to stay current with the latest advancements in hospital care and patient management.

Job Qualifications
  • Valid And Current Nurse Practitioner License In The State Of Florida.

  • Master's Degree In Nursing From An Accredited Program.

  • Minimum Of 2 Years Experience As A Nurse Practitioner In A Hospital Setting.

  • Certification As A Nurse Practitioner In The Specialty Of Family Or Adult-Gerontology.

  • Strong Clinical Skills And Ability To Work Independently In A Fast-Paced And Dynamic Environment.
