- Oversee the coordination and management of patient care to ensure high-quality and effective services are provided.

- Lead a team of care management professionals and provide guidance, support, and feedback to ensure their success.
- Monitor and evaluate the performance of the care management team, identifying areas for improvement and implementing corrective actions as needed.
- Collaborate with other healthcare professionals to create and implement care plans that meet the individual needs of each patient.
- Ensure compliance with all regulatory and accreditation standards related to patient care.

- Act as a liaison between patients, families, and healthcare providers to address and resolve any concerns or issues related to patient care.
- Promote a patient-centered approach to care management and advocate for the needs and rights of patients.
- Represent the organization in a professional manner and uphold its mission, vision, and values at all times.
Bachelor's Degree In Nursing, Healthcare Administration, Or Related Field.
Minimum Of 5 Years Of Experience In Healthcare Management Or Care Coordination.
Strong Leadership Skills And Ability To Effectively Manage And Develop A Team.
Knowledge Of Healthcare Regulations And Compliance, Including Medicare And Medicaid.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Various Stakeholders And Departments.
