- Develop and implement personalized care plans for patients with heart failure, based on their individual needs and treatment goals.
- Work closely with physicians and other members of the healthcare team to coordinate and manage the care of heart failure patients.
- Provide education and guidance to patients and their families on heart failure management, including medication adherence, lifestyle modifications, and symptom recognition.
- Monitor and evaluate patient progress and adjust care plans accordingly to ensure optimal outcomes.
- Serve as a resource for patients and their families, addressing any questions or concerns they may have regarding their condition or treatment.
- Collaborate with community resources and support services to ensure patients have access to necessary resources for their care.
- Maintain accurate and up-to-date patient records and documentation of care provided.
- Participate in quality improvement initiatives to enhance the care and experience of heart failure patients.
- Stay current on advancements and developments in the field of cardiology and heart failure management.
- Uphold the values and standards of Orlando Health by providing compassionate and patient-centered care at all times.
Registered Nurse (Rn) With A Current License In The State Of Florida
Minimum Of 2 Years Of Experience In Cardiovascular Nursing, With A Focus On Heart Failure Management
Certification In Heart Failure Nursing, Such As Certified Heart Failure Nurse (Chfn) Or Advanced Heart Failure Certified Nurse (Ahfcn)
Knowledge And Experience With Evidence-Based Heart Failure Treatment And Management Protocols
Excellent Communication And Interpersonal Skills, With The Ability To Educate And Empower Patients And Their Families In Self-Care Management Of Heart Failure
