- Coordinate and facilitate communication between patients, families, and healthcare providers to ensure a holistic approach to care.
- Develop and maintain relationships with patients and their families to understand their needs and preferences.
- Educate and support families in understanding treatment plans, procedures, and resources available to them.
- Collaborate with healthcare team members to develop personalized care plans for patients and their families.
- Provide emotional support and counseling to families during difficult times and help them navigate the healthcare system.
- Advocate for patients and their families to ensure their needs and concerns are addressed.
- Conduct regular check-ins and follow-ups with families to assess their satisfaction and make necessary adjustments to care plans.
- Maintain accurate and up-to-date records of patient and family interactions.
- Participate in team meetings and provide input on ways to improve family-centered care.
- Stay informed and up-to-date on industry trends and best practices related to family-centered care.
- Adhere to all policies, procedures, and regulations regarding patient privacy and confidentiality.
- Serve as a liaison between the hospital and community organizations to provide additional support and resources to families.
- Collaborate with other departments to plan and implement family-centered events and programs.
- Continuously evaluate and improve the effectiveness of family-centered care at Orlando Health.
Bachelor's Degree In Social Work, Counseling, Or A Related Field.
Minimum Of 3 Years Experience In Case Management Or Family Advocacy.
Knowledge Of Community Resources And Services For Families, Including Health Care, Mental Health, And Social Services.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Families And Healthcare Professionals.
Experience Working With Diverse Populations And Sensitivity To Cultural Differences.
