  1. Ensure the safe and effective use of medications for patients by reviewing orders and verifying accuracy, appropriateness, and potential interactions.
  2. Provide evidence-based recommendations for medication therapy to healthcare teams, including physicians, nurses, and other healthcare professionals.
  3. Collaborate with other healthcare team members to develop comprehensive treatment plans and ensure proper medication management for patients.
  4. Monitor medication therapy and patient response to identify and resolve any potential issues or adverse reactions.
  5. Educate patients and their caregivers on proper medication use, potential side effects, and any necessary precautions.
  6. Maintain accurate and complete patient records, including medication histories and drug interactions.
  7. Stay up-to-date on current pharmaceutical practices, new medications, and healthcare regulations to provide the best care for patients.
  8. Work closely with pharmacy technicians to ensure accurate and timely preparation and dispensing of medications.
  9. Participate in quality improvement initiatives to enhance patient outcomes and medication safety.
  10. Adhere to all hospital policies, procedures, and regulations, including those related to medication use and storage.
  11. Act as a resource for other healthcare professionals, answering questions and providing guidance on medication-related issues.
  12. Maintain a professional and compassionate demeanor when interacting with patients, their families, and healthcare team members.
  13. Communicate effectively with patients, their families, and healthcare team members to ensure proper understanding of medication therapy and treatment plans.
  14. Participate in continuing education and professional development opportunities to enhance knowledge and skills in clinical pharmacy practice.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution

  • Active Pharmacist License In The State Of Florida

  • Minimum Of 2 Years Of Experience As A Clinical Pharmacist In A Hospital Setting

  • Board Certification In Pharmacotherapy (Bcps) Or Other Relevant Specialty (E.g. Oncology, Critical Care)

  • Strong Knowledge Of Current Pharmacy Laws, Regulations, And Best Practices In Patient Care And Medication Management.
