If so, we have an exciting opportunity for you as a Case Manager for our Teen Xpress program at Orlando Health. In this role, you will be responsible for providing case management services to at-risk teens, helping them navigate through social, emotional, and behavioral challenges. Your compassionate and supportive approach will help these teens achieve their full potential and lead fulfilling lives. We are looking for candidates with a Bachelor's degree in social work, psychology, or a related field, and at least 2 years of experience working with adolescents. Strong communication, organizational, and problem-solving skills are essential for success in this role.   1. Conduct thorough assessments of at-risk teens to identify their individual needs and develop personalized treatment plans.
  2. Establish and maintain regular contact with clients, providing ongoing support and monitoring progress.
  3. Collaborate with other professionals, such as therapists and medical providers, to coordinate comprehensive care for clients.
  4. Advocate for clients and connect them with necessary resources, such as housing, employment, and educational opportunities.
  5. Monitor and document client progress and make necessary adjustments to treatment plans.
  6. Provide crisis intervention and support during times of heightened need.
  7. Develop and facilitate group therapy sessions and educational workshops for clients.
  8. Maintain a deep understanding of local resources and services available to at-risk teens.
  9. Collaborate with families and caregivers to ensure a holistic approach to client care.
  10. Attend regular meetings and trainings to stay up-to-date on best practices and program updates.
  11. Maintain accurate and confidential client records in compliance with organizational and legal standards.
  12. Uphold ethical and professional standards in all interactions with clients and colleagues.
  13. Participate in program evaluation and quality improvement initiatives.
  14. Serve as a positive role model and mentor for the clients.
  15. Continuously seek out opportunities for professional development and growth in the field of social work and counseling.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or Related Field.

  • Minimum Of 2 Years Experience Working With Adolescents In A Healthcare Or Social Services Setting.

  • Knowledge Of Community Resources And Services For At-Risk Teens.

  • Strong Communication And Interpersonal Skills To Effectively Work With Teen Clients And Their Families.

  • Ability To Work Independently And As Part Of A Multidisciplinary Team To Develop And Implement Individualized Care Plans For Teen Clients.
