- Collaborate with healthcare professionals, including therapists, social workers, and physicians, to coordinate and monitor patient care.
- Facilitate communication and coordination between patients, families, and healthcare providers to ensure the best possible outcomes.
- Advocate for patients to receive appropriate and timely treatment and services.
- Conduct regular assessments and reassessments of patients to monitor progress and identify any changes or adjustments needed in their care plans.
- Provide education and support to patients and their families to promote understanding and compliance with treatment plans.
- Work closely with insurance providers to ensure coverage for necessary treatments and services.
- Maintain accurate and thorough documentation of patient interactions, progress, and treatment plans.
- Stay current with industry trends and developments in the field of behavioral health in order to provide the best possible care for patients.
- Collaborate with community resources, such as support groups and social services, to provide additional support and resources for patients.
- Display empathy and compassion towards patients and their families, while maintaining professional boundaries.
- Adhere to all legal and ethical standards and maintain confidentiality of patient information.
- Participate in team meetings and case conferences to discuss patient progress and make recommendations for adjustments to treatment plans.
- Continuously strive for excellence in patient care and seek opportunities for professional development.
- Maintain a positive and collaborative working relationship with all members of the healthcare team.
Strong Communication And Interpersonal Skills.
Bachelor's Or Master's Degree In Social Work, Psychology, Or A Related Field.
Minimum Of 2 Years Experience In A Behavioral Health Or Mental Health Setting.
Knowledge Of Case Management Principles And Experience With Care Coordination.
Ability To Work With A Diverse Population And Collaborate With Other Healthcare Professionals.
