
Care Management Educator

- Develop and implement educational programs for patients and their families to promote health literacy and self-management skills.
- Train and support the care management team on patient education techniques and resources.
- Collaborate with healthcare providers to identify educational needs for patients and develop appropriate materials.
- Conduct patient education sessions and provide one-on-one education as needed.
- Evaluate the effectiveness of educational programs and make necessary improvements.
- Ensure all patient education materials are up-to-date, accurate, and accessible.
- Maintain knowledge of current healthcare trends and best practices in patient education.
- Serve as a resource for patients and families, providing guidance and support in navigating the healthcare system.
- Work closely with interdisciplinary teams to develop individualized care plans for patients.
- Maintain accurate documentation and records of patient interactions and educational sessions.
- Participate in quality improvement initiatives to enhance patient education and care management services.
- Stay informed of insurance coverage and reimbursement policies related to patient education services.
- Demonstrate compassion and empathy towards patients and their families while educating and empowering them.
- Foster a positive and collaborative work environment within the care management team.
- Adhere to all ethical and legal standards in delivering patient education services.
Excellent Communication And Interpersonal Skills
Bachelor's Degree In Nursing Or A Related Field
Minimum Of 3-5 Years Experience In Care Management Or Healthcare Education
Strong Knowledge Of Care Management Principles And Best Practices
Previous Experience In Developing And Delivering Educational Programs Or Courses Preferred
