Orlando Health is seeking a dedicated Care Management Educator to join our team and help us in our mission to provide exceptional care and education to our patients. In this role, you will be responsible for developing and implementing educational programs for patients and their families, as well as training and supporting our care management team. We are looking for someone who is compassionate, knowledgeable, and has excellent communication and organizational skills.   1. Develop and implement educational programs for patients and their families to promote health literacy and self-management skills.
  2. Train and support the care management team on patient education techniques and resources.
  3. Collaborate with healthcare providers to identify educational needs for patients and develop appropriate materials.
  4. Conduct patient education sessions and provide one-on-one education as needed.
  5. Evaluate the effectiveness of educational programs and make necessary improvements.
  6. Ensure all patient education materials are up-to-date, accurate, and accessible.
  7. Maintain knowledge of current healthcare trends and best practices in patient education.
  8. Serve as a resource for patients and families, providing guidance and support in navigating the healthcare system.
  9. Work closely with interdisciplinary teams to develop individualized care plans for patients.
  10. Maintain accurate documentation and records of patient interactions and educational sessions.
  11. Participate in quality improvement initiatives to enhance patient education and care management services.
  12. Stay informed of insurance coverage and reimbursement policies related to patient education services.
  13. Demonstrate compassion and empathy towards patients and their families while educating and empowering them.
  14. Foster a positive and collaborative work environment within the care management team.
  15. Adhere to all ethical and legal standards in delivering patient education services.

Job Qualifications
