- Provide comprehensive care coordination services to patients in need.
- Work closely with healthcare professionals to develop and implement individualized care plans for patients.
- Conduct assessments to identify patient needs and develop appropriate interventions.

Bachelor's Degree In Social Work Or Related Field: A Care Coordinator, Social Worker Ii Should Have A Minimum Of A Bachelor's Degree In Social Work Or A Related Field Such As Psychology Or Sociology. This Educational Background Provides The Necessary Knowledge And Skills To Effectively Assess And Address The Needs Of Patients.
State Licensure As A Social Worker: The Candidate Should Possess A Valid License As A Social Worker In The State Of Florida. This Ensures That The Individual Has Met The Necessary Requirements And Has The Necessary Training To Provide Social Work Services.
Minimum Of 2 Years Experience In A Healthcare Setting: The Ideal Candidate Should Have At Least 2 Years Of Experience Working In A Healthcare Setting, Preferably In A Case Management Or Care Coordination Role. This Experience Makes Them Familiar With The Healthcare System And The Resources Available To Patients.
Knowledge Of Community Resources: A Care Coordinator, Social Worker Ii Should Have A Strong Understanding Of The Community Resources And Services Available To Patients. This Knowledge Allows Them To Effectively Connect Patients With The Necessary Support And Assistance.
