  1. Coordinate and manage the care of patients at Orlando Health, ensuring that their needs are met with the highest quality of care.
  2. Work closely with a multidisciplinary team to develop and implement care plans that meet the individual needs of each patient.
  3. Advocate for patients and their families, ensuring that their voices are heard and their concerns are addressed.
  4. Communicate effectively with patients, families, and healthcare providers to provide timely and accurate information regarding care plans, treatment options, and resources.
  5. Utilize critical thinking skills to identify and address any barriers to care or potential risks to patient health.
  6. Collaborate with community resources and support services to facilitate a seamless transition of care for patients.
  7. Monitor and evaluate patient progress, identifying any areas for improvement or additional support.
  8. Educate patients and their families about their healthcare options and provide guidance on managing their conditions.
  9. Maintain accurate and up-to-date documentation of patient care plans, progress, and any changes in their condition.
  10. Adhere to all legal and ethical standards in providing care to patients.
  11. Participate in ongoing training and professional development opportunities to stay current with industry standards and advancements in healthcare.
  12. Promote a culture of excellence and patient-centered care within the organization.
  13. Demonstrate empathy and compassion in all interactions with patients and their families.
  14. Serve as a liaison between patients, families, and healthcare providers to ensure open communication and facilitate a positive patient experience.
  15. Prioritize and manage a high volume of patients, while maintaining attention to detail and accuracy in all aspects of care coordination.

Job Qualifications
  • A Valid Registered Nurse (Rn) License In The State Of Florida.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In A Case Management Or Care Coordination Role.

  • Strong Knowledge Of Healthcare Systems, Insurance Policies, And Utilization Management Protocols.

  • Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Families, And Interdisciplinary Healthcare Teams.

  • Ability To Multitask, Prioritize, And Problem-Solve In A Fast-Paced And Dynamic Healthcare Environment.
