- Coordinate care for patients in the Behavioral Health department, ensuring they receive the highest quality of care and support.
- Collaborate with patients, their families, and interdisciplinary teams to develop and implement individualized care plans.
- Conduct assessments, evaluate patient needs, and make appropriate referrals to resources and support services.
- Monitor and track patient progress and make necessary adjustments to care plans.
- Serve as a liaison between patients, providers, and community resources.
- Educate patients and their families on available resources and treatment options.
- Advocate for patients' needs and ensure they receive appropriate and timely care.
- Maintain accurate and up-to-date patient records and documentation.
- Stay current with industry standards and best practices in Behavioral Health care.
- Provide emotional support and guidance to patients and their families during difficult times.
- Communicate effectively with team members and other departments to ensure continuity of care.
- Participate in staff meetings, trainings, and other professional development opportunities.
- Adhere to all legal and ethical standards and maintain patient confidentiality.
- Uphold the mission and values of Orlando Health in all interactions with patients and colleagues.
Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or A Related Field.
Minimum Of 3-5 Years Of Experience In A Behavioral Health Setting, Preferably In A Care Coordination Or Case Management Role.
Knowledge Of Mental Health And Substance Abuse Disorders, Treatment Modalities, And Community Resources.
Strong Communication And Interpersonal Skills With The Ability To Establish Rapport And Collaborate With Patients, Families, And Interdisciplinary Team Members.
Familiarity With Electronic Medical Records And Proficiency In Documentation And Data Entry.
