
Nurse Manager, Care Management

- Lead and manage a team of nurses and social workers to provide high-quality, coordinated care for patients.
- Develop and implement strategies to improve patient outcomes and ensure efficient and effective care management.
- Oversee the daily operations of the Care Management team, including staffing, budgeting, and scheduling.
- Promote a culture of compassion, professionalism, and excellence within the team.
- Collaborate with other healthcare professionals to ensure seamless care coordination for patients.
- Monitor and evaluate the performance of team members, providing guidance and support as needed.
- Stay updated on industry best practices and implement evidence-based strategies for care management.
- Serve as a liaison between the Care Management team and other departments within the organization.
- Develop and maintain relationships with external stakeholders, such as insurance companies and community resources.
- Ensure compliance with all relevant healthcare regulations and standards.
- Conduct regular meetings and trainings to educate team members on new policies, procedures, and protocols.
- Handle any conflicts or issues that arise within the team in a professional and timely manner.
- Foster a positive and inclusive work environment where team members feel supported and valued.
- Represent the organization at conferences, seminars, and other events related to care management.
- Continuously seek opportunities to improve and innovate the care management processes within the organization.
Bachelor's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing Is Required For This Position. This Ensures That The Individual Has A Strong Foundation In Nursing Principles And Practices.
Current Rn License In The State Of Oregon: A Valid And Current Registered Nurse (Rn) License In The State Of Oregon Is Necessary For This Role. This License Ensures That The Individual Has Met The State's Requirements For Practicing Nursing.
Minimum Of 3-5 Years Of Experience In A Healthcare Leadership Role: Previous Experience In A Management Or Leadership Role In A Healthcare Setting Is Highly Desirable For This Position. It Shows That The Individual Has The Necessary Skills And Knowledge To Effectively Manage A Team.
Knowledge And Understanding Of Care Management Principles: A Thorough Understanding Of Care Management Principles Is Essential For This Role. This Includes Knowledge Of Evidence-Based Practices, Quality Improvement, And Care Coordination.
