You will work closely with other healthcare providers to ensure our patients receive the comprehensive care that they need and deserve. Responsibilities:

  1. Provide holistic, cost-effective care to patients in their homes.
  2. Coordinate and collaborate with other healthcare providers to ensure comprehensive care.
  3. Manage a diverse array of patients and their needs.
  4. Remain up to date on the changing regulations, guidelines, and standards of care for Nurse Practitioners.
  5. Utilize critical thinking skills to assess, diagnose, and treat patients.
  6. Develop and implement individualized care plans for each patient.
  7. Educate patients and their families on preventative health care and disease management.
  8. Maintain patient records and document all interactions with patients.
  9. Participate in research activities and clinical trials as needed.
  10. Follow all ethical guidelines and standards of care for Nurse Practitioners.

  • Cpr Certification
