- Provide compassionate and holistic care to patients with life-limiting illnesses.
- Utilize advanced practice skills and knowledge to deliver exceptional end-of-life care.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Assess and manage pain and other symptoms to improve quality of life for patients.
- Educate patients and their families on palliative care options and resources.
- Ensure compliance with all relevant healthcare regulations and standards.
- Maintain accurate and up-to-date patient records.
- Continuously evaluate and adjust care plans based on patient needs.
- Participate in patient and family meetings to discuss goals of care and advance care planning.
- Stay current on advancements and best practices in palliative care.
- Demonstrate a high level of professionalism and empathy in all interactions with patients and their families.

- Collaborate with other healthcare providers and community resources to coordinate care for patients.

Advanced Practice Provider (App) Licensure: Candidates For This Prn Palliative Care App Job At Novant Health Must Hold A Valid App License In The State Of Employment.
Master's Degree In Nursing Or Related Field: Applicants Must Possess A Master's Degree From An Accredited Nursing Or Related Program To Be Considered For This Position.
Palliative Care Certification: A Certification In Palliative Care, Such As The Advanced Certified Hospice And Palliative Nurse (Achpn) Or The Certified Hospice And Palliative Care Nurse (Chpn), Is Required For This Prn Position.
Minimum Of 2 Years Of Clinical Experience: Candidates Should Have At Least 2 Years Of Relevant Clinical Experience, Preferably In Palliative Care Or A Related Field.
Excellent Communication And Interpersonal Skills: As This Position Involves Working With Patients And Their Families During A Difficult Time, Strong Communication And Interpersonal Skills Are Essential. Candidates Should Be Able To Demonstrate Empathy, Compassion, And The Ability To Effectively Communicate Complex Medical Information.
