- Accurate and Complete Documentation: Responsible for ensuring accurate and complete documentation of patient data in compliance with state and federal regulations.
- MDS Coordination: Coordinate and oversee the Minimum Data Set process, including assessing, documenting, and analyzing patient data to determine appropriate care plans.
- Quality Patient Care: Play a crucial role in ensuring high-quality patient care by accurately documenting patient data and collaborating with other healthcare team members.
- Organization and Attention to Detail: Maintain a high level of organization and attention to detail in all aspects of the MDS process, including data collection, documentation, and reporting.

- Training and Development: Stay current on industry updates and best practices related to MDS coordination and attend training and development opportunities as needed.
- Process Improvement: Continuously evaluate and improve the MDS process to ensure efficiency and accuracy, leading to improved patient outcomes.
- Patient Advocacy: Serve as an advocate for patients by accurately documenting their data and ensuring that their care needs are met.
Valid Registered Nurse (Rn) License In The State Of Employment
Minimum Of 2 Years Clinical Experience, Preferably In Long-Term Care Or Skilled Nursing Facility Setting
Knowledge Of Medicare And Medicaid Regulations And Requirements For Long-Term Care Facilities
Proficiency In Conducting And Completing Mds Assessments In Accordance With State And Federal Guidelines
Strong Communication And Documentation Skills, With Attention To Detail And Accuracy In Data Collection And Reporting.
