1. Oversee and manage medication management practices for patients at Novant Health.
2. Ensure safe and effective use of medications for patients by following best practices and regulatory guidelines.
3. Collaborate with healthcare team to develop and implement medication management plans for individual patients.
4. Conduct thorough medication reviews and identify any potential drug interactions or adverse effects.
5. Educate patients and their families on proper medication usage, potential side effects, and potential drug interactions.
6. Maintain accurate and up-to-date medication records for patients.
7. Communicate with healthcare providers to address any concerns or issues related to medication management.
8. Monitor and track medication adherence of patients and provide interventions as needed.
9. Participate in quality improvement initiatives to improve medication management practices.
10. Train and mentor pharmacy technicians and other healthcare staff on medication management best practices.
11. Stay up-to-date on current medication management practices and regulatory guidelines.
12. Collaborate with insurance companies to ensure proper coverage for prescribed medications.
13. Utilize electronic medical records and computer systems to document and track medication management activities.
14. Maintain a valid pharmacy technician license and adhere to all state and federal regulations.
15. Demonstrate a strong attention to detail and accuracy in all medication management tasks.
16. Provide exceptional customer service to patients and their families.
17. Drive to various locations to provide medication management services as needed.
18. Work collaboratively with other healthcare professionals, including pharmacists, nurses, and physicians.
