1. Provide compassionate and empathetic care to patients and their families during their end-of-life journey.
2. Collaborate with interdisciplinary team members to develop and implement a personalized care plan for each patient.
3. Monitor and assess patients' physical, emotional, and spiritual needs and provide appropriate interventions.
4. Administer medications and treatments as prescribed by physicians.
5. Educate patients and their families on hospice care and support them through the grieving process.
6. Document patient care and progress accurately and timely.
7. Communicate effectively with physicians, team members, and other healthcare professionals.
8. Maintain a safe and comfortable environment for patients and their families.
9. Advocate for patients' rights and ensure their wishes and preferences are respected.
10. Participate in ongoing education and training to maintain knowledge and skills related to hospice care.
11. Adhere to all hospice regulations, policies, and procedures.
12. Provide emotional support and guidance to patients' families and loved ones.
13. Collaborate with community resources to meet patients' and families' needs.
14. Participate in quality improvement initiatives to ensure the delivery of high-quality care.
15. Serve as a role model and mentor for new hospice team members.
