
Hospice Liaison Nurse - Registered Nurse, RN

- Collaborate with interdisciplinary team members to develop and implement comprehensive care plans for hospice patients.
- Conduct initial assessments of patients' physical, emotional, and spiritual needs and provide ongoing nursing care to ensure their comfort and well-being.
- Serve as a primary point of contact for hospice patients and their families, providing support, guidance, and education throughout the end-of-life process.
- Communicate effectively with healthcare providers, including physicians, social workers, and therapists, to coordinate and optimize patient care.
- Manage and prioritize a caseload of hospice patients, ensuring timely and appropriate interventions and treatments.
- Document patient care in accordance with hospice and regulatory standards, maintaining accurate and up-to-date medical records.
- Provide emotional support and counseling to patients and families, addressing their fears, concerns, and questions about end-of-life care.
- Promote and maintain a safe and comfortable environment for patients, adhering to infection control and safety protocols.
- Conduct regular home visits to assess patient needs and provide hands-on nursing care, including medication administration, wound care, and symptom management.
- Participate in interdisciplinary team meetings to discuss patient progress, identify areas for improvement, and develop individualized care plans.
- Serve as a liaison between hospice patients, their families, and community resources, ensuring access to necessary services and support.
- Stay updated on current hospice care trends and best practices, incorporating them into daily practice to provide high-quality, evidence-based care.
- Maintain a professional and compassionate demeanor at all times, demonstrating empathy, respect, and cultural sensitivity towards patients and their families.
- Adhere to ethical and legal standards of nursing practice, including maintaining patient confidentiality and respecting patients' right to self-determination.
- Collaborate with other hospice liaisons and team members to promote a positive and supportive work environment and achieve organizational goals.
Current Rn Licensure: The Hospice Liaison Nurse Must Hold A Current And Valid Rn License In The State Where They Will Be Working. This Ensures That They Have Met All The Necessary Education And Training Requirements To Practice Nursing.
Previous Hospice Or Palliative Care Experience: A Strong Understanding Of Hospice And Palliative Care Principles Is Essential For This Role. Previous Experience Working In A Hospice Or Palliative Care Setting Is Preferred To Ensure The Nurse Is Knowledgeable About End-Of-Life Care And Able To Provide Appropriate Support And Guidance To Patients And Families.
Excellent Communication And Interpersonal Skills: As A Hospice Liaison Nurse, Effective Communication Is Key To Building Relationships With Patients, Families, And Healthcare Teams. The Nurse Must Be Able To Communicate Compassionately And Effectively With Individuals From Diverse Backgrounds And Demonstrate Empathy Towards Those Facing End-Of-Life Care.
Ability To Work Independently: The Hospice Liaison Nurse Will Be Responsible For Coordinating Care And Services For Patients And Families, Often Working In The Community. As Such, They Must Be Able To Work Autonomously And Be Self-Motivated To Complete Tasks And Meet Deadlines.
Strong Assessment And Critical Thinking Skills: The Hospice Liaison Nurse Must Be Able To Assess Patients' Physical, Emotional, And Psychosocial Needs And Develop Individualized Care Plans. They Should Also Possess Strong Critical Thinking Skills To Prioritize And Make Sound Decisions In Complex And Evolving Situations.
