We are currently seeking a compassionate and knowledgeable Hospice Educator to join our team. In this role, you will have the opportunity to educate and support patients, families, and caregivers during one of the most challenging times in their lives. As a Hospice Educator, you will play a vital role in providing comfort, guidance, and resources to those facing end-of-life care.   1. Develop and implement educational programs and materials for patients, families, and caregivers regarding hospice care and end-of-life planning.
  2. Conduct individual and group education sessions, including in-person and virtual settings, to provide information and support to patients and their loved ones.
  3. Collaborate with interdisciplinary teams to ensure that patients' and families' educational needs are met throughout their hospice journey.
  4. Serve as a resource for patients, families, and caregivers, answering questions and providing guidance on hospice care options and services.
  5. Maintain up-to-date knowledge of hospice care regulations, policies, and procedures, and ensure compliance with all applicable standards.
  6. Utilize effective communication skills to build rapport and trust with patients and families, and to address any concerns or issues that may arise.
  7. Participate in interdisciplinary team meetings and contribute to the development of care plans for patients.
  8. Conduct regular assessments of patients' and families' educational needs and adjust educational plans accordingly.
  9. Foster a compassionate and supportive environment for patients, families, and caregivers during the end-of-life process.
  10. Collaborate with community organizations and healthcare providers to promote awareness and understanding of hospice care.
  11. Document all educational interactions and maintain accurate records of patient and family education.
  12. Continuously evaluate and improve the effectiveness of educational programs and materials.
  13. Participate in ongoing professional development and training opportunities to enhance knowledge and skills related to hospice care.
  14. Adhere to ethical and professional standards of practice in all interactions with patients, families, and colleagues.
  15. Support Novant Health's mission and values by promoting a patient-centered approach and delivering high-quality, compassionate care to those in need.

Job Qualifications
  • Minimum Of A Bachelor's Degree In Education Or A Related Field, With A Focus On Healthcare Education Preferred.

  • At Least 3-5 Years Of Experience In Hospice Care, With A Strong Understanding Of End-Of-Life Care Principles And Practices.

  • Excellent Communication And Presentation Skills, With The Ability To Effectively Educate And Train Both Patients And Healthcare Professionals.

  • Experience In Developing And Implementing Educational Programs, Workshops, And Trainings In A Healthcare Setting.

  • Knowledge Of Current Hospice Regulations And Accreditation Standards, Such As Those Set By Medicare And The Joint Commission.
