The Home Care Services Coordinator will be responsible for providing excellent support to our home care team.The successful candidate must have a minimum of an Associate’s Degree in Healthcare Administration or a related field. Additionally, they must have at least two years of experience working in a home care services role. The Home Care Services Coordinator must also have a valid driver’s license and reliable transportation.The Home Care Services Coordinator will be an integral part of the team, providing essential support to ensure that our patients receive the highest quality of care. Responsibilities:

1. Develop and maintain relationships with patients, families, and other stakeholders in home care services.
2. Coordinate activities of care providers such as home health aides, nurses, and other medical personnel.
3. Monitor patient care plans and provide updates as needed.
4. Assist in creating patient care plans tailored to individual needs.
5. Ensure patient safety and satisfaction with home care services.
6. Maintain accurate records and documentation of care services provided.
7. Assist with scheduling and coordinating services for patients.
8. Monitor patient progress and provide updates to supervisor.
9. Assist in resolving patient and family complaints related to home care services.
10. Provide customer service and support to patients and families.
