
Heart Failure Program Manager

- Develop and implement strategies to improve the quality of care for heart failure patients within the organization.
- Collaborate with interdisciplinary teams to evaluate and improve current heart failure care practices.
- Coordinate and lead meetings with staff and physicians to discuss best practices and provide updates on the heart failure program.
- Establish and maintain relationships with community resources and organizations to enhance patient care and support.
- Develop and maintain a comprehensive education program for healthcare professionals on heart failure management.

- Ensure compliance with regulatory standards and guidelines for heart failure care.
- Oversee the development and implementation of evidence-based protocols and guidelines for heart failure treatment.
- Collaborate with leadership to develop and manage the budget for the heart failure program.
- Serve as a resource for patients and their families, providing education and support on heart failure management and resources.
- Stay updated on industry trends and advancements in heart failure care and implement relevant changes in the program.
- Supervise a team of healthcare professionals and provide guidance and support in their roles within the heart failure program.
- Develop and maintain strong relationships with key stakeholders and partners to enhance the heart failure program's effectiveness.
- Participate in performance improvement initiatives and ensure the program's continuous growth and success.
- Maintain accurate and up-to-date records of program activities, patient outcomes, and budget expenses.
Bachelor's Degree In Nursing, Healthcare Administration, Or Related Field.
Minimum Of 3-5 Years Of Experience In Healthcare Management, Preferably In A Heart Failure Program.
Knowledge And Experience With Evidence-Based Practices For Heart Failure Management.
Strong Leadership And Communication Skills To Effectively Lead A Multidisciplinary Team.
Certification In Heart Failure Management (Such As Chfn Or Chfn-K) Preferred.
