Job Description

Join our team at Novant Health as the Heart Failure Program Manager and lead our efforts in providing exceptional care and support for patients with heart failure. As the Program Manager, you will be responsible for coordinating and implementing initiatives to improve the overall quality of care for our heart failure patients. We are seeking a highly organized and compassionate individual with a background in nursing or healthcare management to join our dedicated team.   1. Develop and implement strategies to improve the quality of care for heart failure patients within the organization.
  2. Collaborate with interdisciplinary teams to evaluate and improve current heart failure care practices.
  3. Coordinate and lead meetings with staff and physicians to discuss best practices and provide updates on the heart failure program.
  4. Establish and maintain relationships with community resources and organizations to enhance patient care and support.
  5. Develop and maintain a comprehensive education program for healthcare professionals on heart failure management.
  6. Monitor and analyze data related to heart failure patient outcomes and identify areas for improvement.
  7. Ensure compliance with regulatory standards and guidelines for heart failure care.
  8. Oversee the development and implementation of evidence-based protocols and guidelines for heart failure treatment.
  9. Collaborate with leadership to develop and manage the budget for the heart failure program.
  10. Serve as a resource for patients and their families, providing education and support on heart failure management and resources.
  11. Stay updated on industry trends and advancements in heart failure care and implement relevant changes in the program.
  12. Supervise a team of healthcare professionals and provide guidance and support in their roles within the heart failure program.
  13. Develop and maintain strong relationships with key stakeholders and partners to enhance the heart failure program's effectiveness.
  14. Participate in performance improvement initiatives and ensure the program's continuous growth and success.
  15. Maintain accurate and up-to-date records of program activities, patient outcomes, and budget expenses.

Job Qualifications
  • Bachelor's Degree In Nursing, Healthcare Administration, Or Related Field.

  • Minimum Of 3-5 Years Of Experience In Healthcare Management, Preferably In A Heart Failure Program.

  • Knowledge And Experience With Evidence-Based Practices For Heart Failure Management.

  • Strong Leadership And Communication Skills To Effectively Lead A Multidisciplinary Team.

  • Certification In Heart Failure Management (Such As Chfn Or Chfn-K) Preferred.
