• Develop personalized nutrition plans for patients based on their medical history, dietary needs, and health goals
• Conduct nutrition assessments and provide education to patients on healthy eating habits and lifestyle changes
• Collaborate with other healthcare professionals to ensure coordinated and comprehensive patient care
• Stay up-to-date on current research and trends in nutrition and incorporate this knowledge into patient care
• Maintain accurate and confidential patient records and documentation
• Communicate effectively with patients, their families, and other members of the healthcare team
• Participate in team meetings and contribute to the development of patient care plans
• Provide support and guidance to patients in adhering to their nutrition plans
• Act as a resource for patients and their families regarding nutrition-related questions and concerns
