- Identify and connect with community members in need of healthcare resources and services.
- Develop and maintain relationships with community leaders and organizations to promote access to healthcare.
- Conduct outreach and education programs to raise awareness of available healthcare resources and services.
- Provide culturally sensitive and appropriate support to individuals and families in navigating the healthcare system.
- Assist community members in accessing necessary healthcare services, such as scheduling appointments and obtaining insurance coverage.
- Conduct home visits and provide in-person support to individuals who may have difficulty accessing healthcare services.
- Collaborate with healthcare providers to ensure that community members receive appropriate and timely care.
- Maintain accurate and detailed records of community interactions and services provided.
- Participate in community events and initiatives to promote health and wellness.
- Continuously evaluate and assess the needs of the community and make recommendations for improvement.
- Serve as a liaison between community members and healthcare organizations to address any barriers to care.
- Adhere to all ethical and legal standards in the provision of healthcare services.
- Attend trainings and workshops to stay informed on current healthcare policies and best practices in community health.
- Represent the organization in a professional and positive manner at all times.
- Collaborate with other members of the healthcare team to develop and implement strategies for improving community health outcomes.
High School Diploma Or Equivalent: A Minimum Educational Requirement For A Community Health Worker At Novant Health Is A High School Diploma Or Equivalent. This Ensures That Candidates Have A Basic Level Of Education And Are Able To Communicate Effectively With Patients And Other Healthcare Professionals.
Certification In Community Health Work: Novant Health Requires All Community Health Workers To Hold A Valid Certification In Community Health Work. This Certification Ensures That Candidates Have Received Training In Health Promotion, Disease Prevention, And Community Outreach, Which Are Essential Skills For This Role.
Previous Experience In Community Health: Candidates With Previous Experience In Community Health Work Are Preferred For This Role. This Includes Experience In Conducting Health Education Workshops, Organizing Community Health Events, And Providing One-On-One Health Coaching To Individuals.
