Clinical Pharmacist - Critical Care

1. Provide specialized pharmaceutical care to patients in the critical care setting, focusing on improving their health and well-being.
2. Collaborate with healthcare team members to develop and implement treatment plans for patients in critical care.
3. Monitor and review medication orders for appropriateness, safety, and effectiveness.
4. Conduct medication therapy management and ensure proper dosing, administration, and monitoring of medications.
5. Educate patients and their families about medications and potential side effects.
6. Stay up-to-date on current pharmaceutical treatments and advancements in the field of critical care.
7. Maintain accurate and complete patient records, including medication histories and treatment plans.
8. Work closely with other healthcare professionals to ensure safe and effective medication management.
9. Respond to and resolve medication-related queries and concerns from patients and healthcare providers.
10. Adhere to all state and federal regulations and guidelines related to pharmacy practice.
11. Ensure proper storage and handling of medications and supplies.
12. Participate in quality improvement initiatives and assist in developing and implementing best practices related to pharmaceutical care in critical care.
13. Serve as a resource and mentor for other pharmacists and healthcare professionals.
14. Maintain a high level of professionalism and ethical standards in all interactions with patients, colleagues, and healthcare providers.
