Clinical Pharmacist Ambulatory(Primary Care)

As an integral member of Novant Health Primary Care, the Clinical Pharmacist Ambulatory provides pharmaceutical care through the review and verification of prescriptions, patient education, and medication management services to ensure optimal patient outcomes. The successful candidate will have a Doctor of Pharmacy degree and licensure as a pharmacist in the state of practice.This position requires a team-oriented individual with excellent communication and interpersonal skills. The ideal candidate should be highly organized and have the ability to work independently as well as collaboratively in a fast-paced environment. The Clinical Pharmacist Ambulatory must have a strong commitment to evidence-based practice and patient safety. Responsibilities:

1. Review and verify prescriptions to ensure patient safety and optimal outcomes.
2. Educate patients on medications and their proper use.
3. Manage medication therapies and adjust dosages as clinically indicated.

5. Work collaboratively with the multidisciplinary healthcare team to optimize patient care.
6. Maintain accurate and up-to-date patient medical records.
7. Monitor patient progress and response to medications.

9. Ensure compliance with relevant laws, regulations, and standards.
