- Provide comprehensive medication management for patients with cardiovascular conditions, including prescribing, monitoring, and adjusting medications as needed.
- Collaborate with physicians and other healthcare professionals to develop and implement individualized treatment plans for patients.
- Conduct medication education and counseling for patients and their caregivers, ensuring understanding of medication regimens, potential side effects, and proper administration techniques.
- Utilize clinical knowledge and expertise to identify and resolve medication-related problems and ensure safe and effective medication use.

Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy
Valid State Pharmacy License And Registration
Board Certification In Pharmacotherapy Or Ambulatory Care Pharmacy
Minimum Of 2 Years Of Experience In Ambulatory Clinic Or Cardiology Pharmacy Practice
