Job Description

As a Case Manager RN at Novant Health, you will play a vital role in coordinating and providing quality patient care.   1. Coordinate and manage patient care plans in collaboration with physicians, nurses, and other healthcare professionals.
  2. Conduct patient assessments and develop individualized care plans based on patients' medical history, current health status, and specific needs.
  3. Monitor and evaluate the effectiveness of care plans and make necessary adjustments to ensure optimal patient outcomes.
  4. Serve as a patient advocate, ensuring that patients' needs and preferences are addressed and respected throughout their healthcare journey.
  5. Communicate with patients, their families, and healthcare team members to provide education on disease management, treatment options, and available resources.
  6. Facilitate effective communication and coordination among healthcare team members to ensure continuity of care for patients.
  7. Keep accurate and up-to-date records of patient information, including assessments, care plans, progress notes, and discharge plans.
  8. Collaborate with insurance companies, social workers, and community resources to ensure patients have access to necessary services and support.
  9. Stay current on healthcare regulations and policies to ensure compliance and provide quality care to patients.
  10. Participate in interdisciplinary meetings to discuss and review patient cases, provide input, and contribute to the development of best practices.
  11. Continuously evaluate and improve the case management process to enhance patient outcomes and satisfaction.
  12. Act as a mentor and resource to other healthcare team members, providing guidance and support to promote a positive and collaborative work environment.
  13. Maintain confidentiality and adhere to ethical standards in all aspects of patient care.
  14. Attend training and educational programs to stay updated on advancements in healthcare and case management practices.
  15. Demonstrate a commitment to the mission and values of Novant Health and actively contribute to creating a positive and inclusive work culture.

Job Qualifications
  • Active Rn License In The State Of Employment

  • Bachelor's Degree In Nursing (Bsn) From An Accredited Program

  • Minimum Of 2 Years Of Clinical Experience In A Healthcare Setting

  • Certification In Case Management (Ccm) Or Willingness To Obtain Within 1 Year Of Hire

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And Effectively Advocate For Patients' Needs.
