Transplant Social Worker Assistant

Responsibilities:

1. Provide supportive services to transplant patients and their families.
2. Collaborate with members of the healthcare team to ensure patient needs are met.
3. Maintain patient records and ensure confidentiality.
4. Develop and implement effective social work services for transplant patients and families.
5. Utilize effective communication and interpersonal skills to build relationships with patients and families.
6. Educate and advise transplant patients and families about medical, psychological, emotional, and financial issues.
7. Assist in coordinating and supporting patient care activities.
8. Identify, assess, and intervene in crisis situations.
9. Demonstrate cultural competency and sensitivity to patient and family needs.
10. Participate in professional development activities to stay abreast of best practices.
