
Transplant Social Worker Assistant
Northwestern Medicine is looking for an experienced and passionate Transplant Social Worker Assistant to join our team. In this role, you will be responsible for providing support to transplant patients and their families, as well as collaborating with other members of the healthcare team. If you have a strong background in social work and patient care, as well as a compassion and desire to make a difference in people’s lives, we invite you to apply for this position.The ideal candidate for this role will have a master’s degree in social work, at least one year of experience in a healthcare setting, and the ability to provide effective social work services to transplant patients and their families. In addition, you should have excellent communication and interpersonal skills, and an understanding of the psychological, emotional, and financial issues that may be faced by transplant patients and their families. The ability to work effectively within a team and maintain strict confidentiality is essential.
Responsibilities:
- Provide supportive services to transplant patients and their families.
- Collaborate with members of the healthcare team to ensure patient needs are met.
- Maintain patient records and ensure confidentiality.
- Develop and implement effective social work services for transplant patients and families.
- Utilize effective communication and interpersonal skills to build relationships with patients and families.
- Educate and advise transplant patients and families about medical, psychological, emotional, and financial issues.
- Assist in coordinating and supporting patient care activities.
- Identify, assess, and intervene in crisis situations.
- Demonstrate cultural competency and sensitivity to patient and family needs.
- Participate in professional development activities to stay abreast of best practices.
