Job Description

Northwell Health is seeking an experienced and motivated Registered Nurse Case Manager to join our team and provide comprehensive care to patients. The successful candidate will be responsible for managing patient care and coordinating services across a wide range of healthcare settings.The ideal candidate should have a license to practice as a Registered Nurse in New York and a minimum of two years of experience in a case management or related position. We are looking for an individual who is organized, has excellent communication skills, and is able to think critically and problem-solve. Responsibilites:

  1. Manage patient care and coordinate services across multiple healthcare settings.
  2. Develop and implement case management plans that meet the individual needs of each patient.
  3. Monitor patient progress and outcomes to ensure positive results.
  4. Provide support and advice to patients and families to help them better understand their medical care.
  5. Act as a liaison between patients, families, healthcare providers, and other professionals to ensure the delivery of quality care.
  6. Prepare patient discharge plans and follow up with patients to ensure successful transition to the next level of care.
  7. Maintain patient records and documentation in accordance with regulatory standards.
  8. Perform regular assessments, identify health risks, and provide education and resources to patients.
  9. Participate in team meetings and collaborate with other healthcare professionals to develop treatment plans.
  10. Stay up-to-date on medical advancements, treatments, and therapies.
