- Provide direct clinical services to patients and their families, including assessment, counseling, and crisis intervention.
- Develop and implement treatment plans in collaboration with the patient, family, and multidisciplinary team.
- Conduct thorough psychosocial assessments to identify patient needs and barriers to care.
- Advocate for patients and their families to ensure access to necessary resources and services.
- Provide education and support to patients and families on coping strategies and community resources.
- Collaborate with community organizations and agencies to coordinate care and services for patients.
- Maintain accurate and timely documentation of all patient interactions and treatment plans.
- Adhere to ethical and legal standards in all interactions with patients and their families.
- Participate in team meetings and case conferences to discuss patient care and treatment plans.
- Continuously develop and maintain knowledge of current social work principles, practices, and resources.
- Maintain confidentiality and privacy of patient information.
- Serve as a liaison between patients and healthcare team members to ensure comprehensive and coordinated care.
- Provide emotional support and counseling to patients and families during difficult times.
- Collaborate with insurance providers to obtain authorization for necessary services.
- Conduct home visits and outreach to patients who may require additional support.
- Participate in quality improvement initiatives to enhance patient outcomes.
- Serve as an advocate for social justice and equality for patients and their families.
- Comply with all Northwell Health policies, procedures, and regulations.
- Maintain a positive and professional demeanor at all times.
- Act as a role model for ethical and compassionate social work practices.
Master's Degree In Social Work From An Accredited Institution.
Lmsw License From The State Of New York.
Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In A Healthcare Environment.
Strong Knowledge Of Social Work Principles, Practices, And Techniques.
Ability To Work Collaboratively With A Multidisciplinary Team And Provide Effective And Compassionate Care To Patients And Their Families.
