  1. Provide direct clinical services to patients and their families, including assessment, counseling, and crisis intervention.
  2. Develop and implement treatment plans in collaboration with the patient, family, and multidisciplinary team.
  3. Conduct thorough psychosocial assessments to identify patient needs and barriers to care.
  4. Advocate for patients and their families to ensure access to necessary resources and services.
  5. Provide education and support to patients and families on coping strategies and community resources.
  6. Collaborate with community organizations and agencies to coordinate care and services for patients.
  7. Maintain accurate and timely documentation of all patient interactions and treatment plans.
  8. Adhere to ethical and legal standards in all interactions with patients and their families.
  9. Participate in team meetings and case conferences to discuss patient care and treatment plans.
  10. Continuously develop and maintain knowledge of current social work principles, practices, and resources.
  11. Maintain confidentiality and privacy of patient information.
  12. Serve as a liaison between patients and healthcare team members to ensure comprehensive and coordinated care.
  13. Provide emotional support and counseling to patients and families during difficult times.
  14. Collaborate with insurance providers to obtain authorization for necessary services.
  15. Conduct home visits and outreach to patients who may require additional support.
  16. Participate in quality improvement initiatives to enhance patient outcomes.
  17. Serve as an advocate for social justice and equality for patients and their families.
  18. Comply with all Northwell Health policies, procedures, and regulations.
  19. Maintain a positive and professional demeanor at all times.
  20. Act as a role model for ethical and compassionate social work practices.

Job Qualifications
  • Master's Degree In Social Work From An Accredited Institution.

  • Lmsw License From The State Of New York.

  • Minimum Of 2 Years Of Experience In A Clinical Setting, Preferably In A Healthcare Environment.

  • Strong Knowledge Of Social Work Principles, Practices, And Techniques.

  • Ability To Work Collaboratively With A Multidisciplinary Team And Provide Effective And Compassionate Care To Patients And Their Families.
