  1. Provide compassionate and dedicated social work services to patients and their families.
  2. Utilize strong social work skills and knowledge to assess and address the needs of patients and families.
  3. Offer support, advocacy, and resources to individuals and families in need.
  4. Collaborate with interdisciplinary teams to develop and implement a comprehensive treatment plan.
  5. Conduct thorough assessments to identify patients' psychosocial needs and make appropriate referrals.

  7. Utilize evidence-based practices and interventions to promote positive outcomes for patients.
  8. Provide crisis intervention and support to patients and families in times of need.
  9. Stay up-to-date on current social work best practices and research to continuously improve patient care.
  10. Work collaboratively with community agencies and resources to connect patients and families with necessary services.
  11. Participate in case conferences and team meetings to discuss patient progress and develop treatment plans.
  12. Adhere to all ethical standards and guidelines set forth by the profession and Northwell Health.
  13. Act as an advocate for patients and families, ensuring their voices are heard and their needs are met.
  14. Maintain a positive and professional demeanor while interacting with patients, families, and colleagues.

Job Qualifications
  • Master's Degree In Social Work From An Accredited Institution.

  • Current And Valid Lmsw License In The State Of New York.

  • Minimum Of 2 Years Of Experience In A Social Work Role, Preferably In A Healthcare Setting.

  • Knowledge And Understanding Of Relevant State And Federal Laws And Regulations Related To Social Work Practice.

  • Strong Communication, Interpersonal, And Problem-Solving Skills, With The Ability To Work Effectively In A Multidisciplinary Team.
