Homecare Registered Nurse

Northwell Health is looking for a compassionate and experienced Homecare Registered Nurse with a passion for providing top of the line patient care to join our team. The ideal candidate must have a current Registered Nurse license in New York State, as well as a Bachelor’s Degree in Nursing or a related field. Other qualifications include strong clinical assessment skills, excellent communication and organizational abilities, and the ability to work independently and collaboratively with a team. Responsibilities:

• Perform comprehensive assessments of patient’s health and wellbeing
• Develop and implement individualized care plans for patients
• Administer medications and treatments as prescribed
• Monitor patient’s health and adjust care plans as needed
• Educate patients and their families on treatments and disease management
• Provide emotional support to patients and their families
• Collaborate with other healthcare professionals to coordinate the best plan of care
• Document patient care activities and progress according to established standards
• Follow safety protocols and adhere to regulatory requirements
