Job Description

Northwell Health is seeking a highly motivated and experienced Director of Care Coordination to join our team. In this role, you will oversee the development and implementation of care coordination programs, ensuring the delivery of high-quality and efficient care to our patients. As the Director, you will be responsible for managing a team of care coordinators and collaborating with various departments to optimize patient outcomes.   1. Develop and implement care coordination programs to ensure the delivery of high-quality and efficient care to patients.

  3. Collaborate with various departments, including clinical teams, to optimize patient outcomes and ensure seamless coordination of care.

  8. Provide regular reports and updates to senior management on the progress and success of care coordination programs.
  9. Stay informed on industry trends and best practices in care coordination, continuously seeking opportunities for improvement and innovation.

Job Qualifications
  • Minimum Of 5 Years Of Experience In Healthcare Management, With A Focus On Care Coordination And Population Health Management.

  • Bachelor's Degree In Healthcare Administration, Nursing, Or Related Field. Master's Degree Preferred.

  • Knowledge Of And Experience With Healthcare Regulations, Policies, And Procedures.

  • Strong Leadership Skills And The Ability To Effectively Manage A Team Of Care Coordinators.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Various Stakeholders, Including Physicians, Nurses, And Other Healthcare Professionals.
