  1. Develop and implement strategic plans to improve patient outcomes and create seamless care experiences.
  2. Lead and manage a team of care coordination professionals, providing guidance, support, and mentorship.
  3. Oversee and evaluate the performance of care coordination programs, identifying areas for improvement and implementing solutions.
  4. Collaborate with other healthcare professionals and departments to streamline processes and improve communication and coordination.
  5. Stay up-to-date on industry trends and best practices in care coordination to continuously enhance services.
  6. Develop and maintain relationships with external partners and community organizations to enhance care coordination efforts.
  7. Ensure compliance with all relevant regulations and standards.
  8. Identify and implement innovative strategies to improve the quality and efficiency of healthcare services.
  9. Communicate effectively with team members, stakeholders, and other departments to ensure alignment and collaboration.
  10. Analyze data and metrics to track progress, identify areas for improvement, and report on key performance indicators.
  11. Develop and manage department budget, allocating resources appropriately.
  12. Provide leadership and direction to promote a positive and productive work environment.
  13. Act as a liaison between care coordination team and other departments to ensure effective communication and coordination.
  14. Develop and maintain strong relationships with patients and families to promote patient satisfaction and engagement.
  15. Ensure timely and accurate documentation of patient care coordination activities.

Job Qualifications
  • Bachelor's Or Master's Degree In Nursing, Healthcare Administration, Or Related Field.

  • Minimum Of 7 Years Of Experience In Healthcare Management, With At Least 3 Years In Care Coordination Or Case Management.

  • Strong Knowledge Of Healthcare Regulations And Reimbursement Systems.

  • Excellent Leadership And Communication Skills, With The Ability To Collaborate With Interdisciplinary Teams.

  • Experience With Developing And Implementing Care Coordination Programs To Improve Patient Outcomes And Reduce Healthcare Costs.
