  1. Utilize expertise in clinical pharmacy practices to provide high-quality patient care in ambulatory settings.
  2. Develop and implement pharmaceutical care plans for patients, taking into consideration their individual needs and medical history.
  3. Collaborate with healthcare team members to ensure comprehensive and coordinated care for patients.
  4. Stay up-to-date on current medications, treatments, and advancements in the field of clinical pharmacy.
  5. Serve as a resource to other healthcare professionals, providing guidance and advice on medication-related issues.
  6. Conduct medication therapy management (MTM) services, including medication reviews and patient education.
  7. Monitor patient outcomes and adjust treatment plans as needed to ensure optimal results.
  8. Provide leadership and support for pharmacy staff, including training and mentoring.
  9. Maintain accurate and complete patient records, including medication profiles and interventions.
  10. Participate in quality improvement initiatives and research projects to enhance patient care.
  11. Educate patients and their families on proper medication use, potential side effects, and drug interactions.
  12. Adhere to all relevant laws, regulations, and standards related to pharmacy practice.
  13. Continuously evaluate and improve upon clinical pharmacy processes and procedures.
  14. Collaborate with other healthcare professionals to develop and implement evidence-based practice guidelines and protocols.
  15. Act as an advocate for patients, ensuring their needs and concerns are addressed in a timely and compassionate manner.

Job Qualifications
  • Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited Institution

  • Completion Of A Post-Graduate Residency Or Fellowship Program In Ambulatory Care

  • Board Certification In Ambulatory Care Pharmacy (Bcacp) Or Other Relevant Specialty

  • Minimum Of 3-5 Years Of Experience In An Ambulatory Care Setting

  • Strong Clinical Knowledge And Understanding Of Evidence-Based Practices In The Management Of Chronic Diseases And Complex Medication Regimens.
