Doctor Of Pharmacy (Pharm.d.) Degree From An Accredited Institution
Completion Of A Post-Graduate Residency Or Fellowship Program In Ambulatory Care
Board Certification In Ambulatory Care Pharmacy (Bcacp) Or Other Relevant Specialty
Minimum Of 3-5 Years Of Experience In An Ambulatory Care Setting
Strong Clinical Knowledge And Understanding Of Evidence-Based Practices In The Management Of Chronic Diseases And Complex Medication Regimens.
