
Clinical Pharmacist- Ambulatory Oncology

- Develop and implement medication therapy plans for patients in the Ambulatory Oncology department, taking into consideration their unique needs and treatment goals.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure safe and appropriate use of medications and optimize patient outcomes.
- Conduct medication reconciliation and review patient medical records to identify any potential drug interactions or adverse effects.
- Provide drug information and education to patients and their caregivers regarding their treatment and potential side effects.
- Monitor patient responses to medications and adjust therapy as needed to ensure optimal efficacy and safety.
- Participate in multidisciplinary team meetings to discuss patient treatment plans, provide input on medication management, and make recommendations based on the latest evidence-based practices.
- Ensure compliance with all state and federal regulations related to pharmacy practice, including maintaining accurate and complete patient records.
- Keep up to date with the latest advancements and developments in oncology treatments, and apply this knowledge to improve patient care.
- Serve as a resource and mentor to pharmacy staff, students, and residents by providing guidance and support in the area of oncology pharmacy.
- Maintain a high level of professionalism and compassion while interacting with patients and their families, ensuring a positive and supportive experience during their cancer journey.
- Participate in quality improvement initiatives and assist in the development and implementation of new pharmacy programs and services.
- Contribute to the overall success of the Ambulatory Oncology department by actively participating in departmental meetings, committees, and projects.
- Adhere to Northwell Health's policies and procedures, including those related to patient safety, confidentiality, and ethical standards.
- Maintain a clean and organized work environment and ensure proper storage and handling of medications and supplies.
- Continuously evaluate and improve pharmacy processes and procedures to enhance the delivery of patient-centered care.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School
Active Pharmacist License In The State Of New York
Board Certification In Oncology Pharmacy (Bcop)
Previous Experience In Ambulatory Oncology Pharmacy Practice
