  1. Provide compassionate and high-quality patient-centered care for transplant patients at MUSC Health.
  2. Serve as a key member of the healthcare team, working collaboratively with physicians, surgeons, and other healthcare professionals to ensure optimal patient outcomes.
  3. Coordinate and manage all aspects of the patient's transplant journey, from pre-transplant evaluation to post-operative care and follow-up.
  4. Conduct thorough assessments and develop individualized care plans for transplant patients based on their unique needs and medical history.
  5. Monitor and manage patients' physical and emotional well-being, identifying and addressing any potential complications or concerns.
  6. Educate patients and their families about the transplant process, including pre- and post-operative care, medications, and potential risks and benefits.
  7. Act as a liaison between patients, their families, and the healthcare team, providing ongoing support and communication throughout the transplant process.
  8. Ensure accurate and timely documentation of all patient care activities in accordance with hospital policies and procedures.
  9. Stay up-to-date with the latest research and best practices in transplant care to continuously improve patient outcomes.
  10. Demonstrate a strong commitment to patient advocacy, promoting and maintaining the highest standards of patient safety and quality of care.

Job Qualifications
  • Current Rn License: The Transplant Rn Coordinator Must Have A Current And Active Rn License In The State Of South Carolina.

  • Transplant Experience: A Minimum Of 2 Years Of Experience Working In A Transplant Setting Is Required For This Position. This Can Include Experience In A Transplant Clinic, Hospital Unit, Or Other Related Area.

  • Knowledge Of Transplant Procedures And Protocols: The Transplant Rn Coordinator Must Have A Thorough Understanding Of The Transplant Process, Including Pre- And Post-Operative Care, Immunosuppressive Therapy, And Potential Complications.

  • Strong Communication And Interpersonal Skills: This Position Involves Working Closely With Patients, Physicians, And Other Healthcare Professionals. The Transplant Rn Coordinator Must Have Excellent Communication And Interpersonal Skills To Effectively Educate And Support Patients And Their Families.

  • Organizational And Time-Management Skills: The Transplant Rn Coordinator Is Responsible For Coordinating Multiple Aspects Of A Patient's Transplant Journey, Including Scheduling Appointments, Coordinating Tests And Procedures, And Ensuring Timely Communication With All Members Of The Transplant Team. Strong Organizational And Time-Management Skills Are Essential For Success In This Role.
