As a Social Worker in our department, you will play a crucial role in supporting and advocating for the physical, emotional, and social well-being of our adolescent patients.   1. Conduct comprehensive psychosocial assessments of adolescent patients to identify their needs and develop appropriate care plans.
  2. Provide individual and family counseling services to address emotional and behavioral health concerns of adolescent patients.
  3. Collaborate with the interdisciplinary team to develop and implement treatment plans that address the physical, emotional, and social needs of adolescent patients.
  4. Advocate for the rights and needs of adolescent patients and their families within the healthcare system.
  5. Educate adolescent patients and their families on available resources and support services.
  6. Monitor and evaluate the progress of adolescent patients and make necessary adjustments to treatment plans.
  7. Maintain accurate and timely documentation of patient assessments, interventions, and progress.
  8. Participate in multidisciplinary team meetings and case conferences to discuss patient care and develop coordinated approaches.
  9. Conduct outreach and community education initiatives to promote adolescent health and address social determinants of health.
  10. Stay updated on current research and best practices in adolescent health and social work.
  11. Adhere to ethical standards and maintain confidentiality of patient information.
  12. Collaborate with community organizations and agencies to coordinate services and support for adolescent patients.
  13. Communicate effectively with patients, families, and other healthcare professionals.
  14. Participate in professional development opportunities and continuous learning to enhance skills and knowledge.
  15. Uphold the values and mission of MUSC Health in all interactions and responsibilities.

Job Qualifications
  • Master's Degree In Social Work From An Accredited Program.

  • State Licensure As A Social Worker (Lmsw Or Lcsw).

  • Minimum Of 2-3 Years Of Experience Working With Adolescents In A Healthcare Or Mental Health Setting.

  • Knowledge And Understanding Of Adolescent Development And Behavior.

  • Strong Communication And Interpersonal Skills, With The Ability To Work Effectively With A Multidisciplinary Team And Diverse Patient Population.
