  1. Coordinate and facilitate the admission and transfer of patients to and from MUSC Health facilities.
  2. Communicate effectively with patients, families, physicians, and healthcare team members to ensure a smooth transition of care.
  3. Conduct thorough assessments of patients' medical history, current condition, and care needs to determine appropriate placement and level of care.
  4. Collaborate with other healthcare professionals to develop and implement individualized care plans for each patient.
  5. Monitor and evaluate patients' progress, and make changes to care plans as needed.
  6. Ensure all necessary paperwork, documentation, and authorizations are completed accurately and in a timely manner.
  7. Serve as a resource for patients and their families, providing education and support throughout the admission and transfer process.
  8. Maintain a safe and clean environment for patients and staff, adhering to infection control and safety protocols.
  9. Adhere to all legal and ethical standards of nursing practice.
  10. Participate in ongoing education and training to maintain and enhance clinical skills and knowledge.
  11. Act as a patient advocate, ensuring that their rights and preferences are respected and addressed.
  12. Collaborate with other members of the healthcare team to identify and resolve any issues or concerns related to patient care.
  13. Maintain confidentiality of patient information and adhere to HIPAA guidelines.
  14. Demonstrate a positive and compassionate attitude towards patients, families, and colleagues.
  15. Follow all MUSC Health policies and procedures to provide high-quality and safe patient care.

Job Qualifications
  • Bsn Degree From An Accredited Nursing Program: A Bachelor Of Science In Nursing (Bsn) Is Typically Required For A Rn Position At Musc Health. This Degree Provides A Strong Foundation In Nursing Theory, Clinical Skills, And Evidence-Based Practice.

  • Current Rn License In The State Of South Carolina: All Rns At Musc Health Must Hold A Valid And Unrestricted License To Practice Nursing In The State Of South Carolina. This Ensures That Candidates Have Met The Necessary Educational And Competency Requirements To Provide Safe And Effective Care.

  • Minimum Of 2 Years Of Nursing Experience: The Admit Transfer Center At Musc Health Requires Rns To Have At Least 2 Years Of Clinical Nursing Experience. This Demonstrates A Strong Understanding Of Nursing Principles And The Ability To Provide Quality Patient Care.

  • Excellent Communication And Critical Thinking Skills: As An Admit Transfer Center Rn, You Will Be Responsible For Coordinating And Facilitating Patient Transfers Between Facilities. Strong Communication And Critical Thinking Skills Are Essential To Effectively Communicate With Healthcare Teams And Make Quick And Accurate Decisions In A Fast-Paced Environment.

  • Knowledge Of Medical Terminology And Patient Care Procedures: Musc Health Expects Their Rns To Have A Strong Understanding Of Medical Terminology And Patient Care Procedures. This Includes Knowledge Of Disease Processes, Treatment Modalities, And Medications. This Knowledge Is Crucial For Providing Safe And Effective Care To Patients In The Admit Transfer Center.
