  1. Provide high-quality and compassionate care to patients in their homes.
  2. Assess and develop care plans for patients based on their individual needs.
  3. Coordinate and collaborate with other healthcare professionals to ensure the best possible outcomes for patients.
  4. Monitor and document patient progress and make necessary adjustments to care plans.
  5. Educate patients and their families on proper self-care and disease management techniques.
  6. Advocate for patients and ensure their needs and preferences are being met.
  7. Maintain accurate and up-to-date patient records.
  8. Follow all protocols and guidelines set by MUSC Health and regulatory bodies.
  9. Communicate effectively with patients, families, and other healthcare team members.
  10. Participate in ongoing education and training to maintain and improve skills and knowledge.
  11. Adhere to ethical and legal standards in patient care.
  12. Collaborate with interdisciplinary team to coordinate and provide comprehensive care.
  13. Develop and maintain positive relationships with patients and their families.
  14. Maintain confidentiality and professionalism in all interactions.
  15. Continuously evaluate and improve processes to enhance patient care and outcomes.

Job Qualifications

  • Bachelor's Degree In Nursing: A Registered Nurse Case Manager At Musc Health Must Have A Bachelor's Degree In Nursing From An Accredited Institution. This Provides A Strong Foundation In Both Theoretical Knowledge And Clinical Skills Necessary For The Role.

  • Current Rn License In State Of Practice: The Candidate Must Hold A Current And Active Registered Nurse License In The State Of Practice. This Ensures That The Individual Is Qualified To Provide Nursing Care And Can Practice Legally Within The State.

  • Minimum Of 2 Years Of Clinical Experience: Musc Health Requires A Minimum Of 2 Years Of Clinical Experience As An Rn, Preferably In A Home Health Or Community Health Setting. This Ensures That The Candidate Has A Solid Understanding Of Nursing Practices And Is Prepared To Take On The Responsibilities Of A Case Manager.

  • Certification In Case Management: While Not Required, Musc Health Prefers Candidates Who Are Certified In Case Management. This Certification Demonstrates A Higher Level Of Knowledge And Expertise In The Field, And Can Enhance The Candidate's Ability To Effectively Manage Patient Cases.

  • Strong Communication And Interpersonal Skills: A Registered Nurse Case Manager At Musc Health Must Possess Excellent Communication And Interpersonal Skills To Effectively Collaborate With Patients, Their Families, And Other Healthcare Professionals. This Includes The Ability To Listen Actively, Provide Clear And Concise Instructions, And Build Rapport With Patients And Their Support Network.
