We are currently seeking a Pre-Kidney Transplant Program Specialist to join our dynamic team and assist in the management of our pre-kidney transplant program. This role requires a driven and experienced healthcare professional who can effectively coordinate and navigate the complex processes involved in the pre-transplant evaluation process.   1. Coordinate and manage the pre-kidney transplant program, ensuring smooth and efficient processes for patients and healthcare providers.
  2. Work closely with patients and their families to provide education and support throughout the pre-transplant evaluation process.
  3. Collaborate with interdisciplinary teams to ensure comprehensive and individualized patient care.
  4. Serve as a point of contact for patients, answering questions and addressing concerns related to the pre-kidney transplant program.
  5. Maintain accurate and up-to-date records of patient evaluations, test results, and other pertinent information.
  6. Conduct thorough evaluations of potential transplant candidates, including reviewing medical records and coordinating necessary tests and procedures.
  7. Facilitate communication and coordination between patients, healthcare providers, and insurance companies.
  8. Monitor and track patients' progress through the pre-transplant evaluation process, identifying and addressing any potential barriers.
  9. Stay current on industry standards and best practices related to pre-transplant evaluation and transplantation.
  10. Collaborate with other healthcare professionals to develop and implement strategies to improve the pre-kidney transplant program.
  11. Maintain strict confidentiality of patient information and adhere to all HIPAA regulations.
  12. Participate in continuing education and training opportunities to enhance knowledge and skills related to pre-transplant evaluation and transplantation.
  13. Serve as a patient advocate, promoting patient rights and ensuring high-quality care.
  14. Assist with the development and implementation of policies and procedures related to the pre-kidney transplant program.
  15. Perform other duties and responsibilities as assigned by the program director or supervisor.

Job Qualifications
