
Pediatric Clinical Pharmacist - Inpatient/Clinic Pharmacy

- Conduct comprehensive medication reviews and assessments for pediatric patients to ensure the safe and effective use of medications.
- Collaborate with physicians, nurses, and other healthcare professionals to develop individualized treatment plans for pediatric patients.
- Provide specialized pharmaceutical care to pediatric patients in both inpatient and clinic settings.
- Monitor medication therapy and make necessary adjustments to dosage or medication regimen to ensure optimal outcomes for pediatric patients.
- Educate patients, families, and caregivers about medication therapy and proper administration techniques.
- Stay up-to-date on the latest developments and guidelines in pediatric pharmacy to provide the best possible care to patients.
- Participate in interdisciplinary rounds and provide medication-related recommendations to optimize patient outcomes.
- Maintain accurate and complete medication records and ensure compliance with regulatory standards.
- Work closely with the pharmacy team to ensure appropriate medication inventory levels for pediatric patients.
- Serve as a resource to healthcare professionals and provide guidance on pediatric medication-related issues.
- Help develop and implement protocols and procedures for safe medication management in the pediatric population.
- Adhere to all hospital policies, procedures, and standards of practice.
- Continuously seek opportunities for professional growth and development to enhance pediatric pharmacy knowledge and skills.
- Maintain confidentiality of patient information and adhere to HIPAA regulations.
- Foster a culture of safety and quality within the pediatric pharmacy department and throughout the organization.
Current Pharmacist License In The State Of South Carolina
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution
Residency Training Or Equivalent Experience In Pediatric Pharmacy Practice
Board Certification In Pediatric Pharmacy (Bcpp) Or Equivalent Certification
Knowledge Of Pediatric Pharmacotherapy, Including Dosing And Monitoring For Various Age Groups And Disease States
