- Provide specialized pharmaceutical care to pediatric patients and their families.
- Collaborate with healthcare team to develop and implement treatment plans for pediatric patients.
- Conduct medication reviews and assessments to ensure safe and effective drug therapy.
- Monitor patient responses to medication and make necessary adjustments.
- Educate patients and their families on medication usage, side effects, and potential interactions.
- Stay up-to-date on current research and developments in pediatric pharmacotherapy.
- Maintain accurate and detailed patient records.
- Adhere to all regulatory guidelines and laws related to pharmaceutical care.
- Participate in multidisciplinary rounds and meetings to discuss patient care.
- Work closely with physicians and other healthcare providers to optimize patient outcomes.
- Provide guidance and mentorship to pharmacy students and residents.
- Serve as a resource for staff and patients regarding drug information and pediatric pharmacotherapy.
- Advocate for patient safety and medication error prevention.
- Continuously assess and improve pharmaceutical care processes.
- Participate in ongoing professional development and education opportunities.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy School.
Completion Of A Pgy-2 Residency In Pediatrics Or Equivalent Experience.
Board Certification In Pediatric Pharmacy (Bcpp) Or Board Certified Pharmacotherapy Specialist (Bcps) With Pediatric Subspecialty Certification.
At Least 2 Years Of Experience Working As A Clinical Pharmacist In A Pediatric Setting.
Strong Knowledge Of Pediatric Pharmacotherapy And Drug Dosing, As Well As Experience With Pediatric Medication Safety And Adherence Programs.
