  1. Coordinate and manage the care of patients within the Patient-Centered Medical Home (PCMH) model.
  2. Collaborate with interdisciplinary team members to develop and implement comprehensive care plans for patients.
  3. Monitor and track patient progress, ensuring timely and appropriate follow-up care.
  4. Serve as the primary point of contact for patients and their families, addressing any questions or concerns they may have.
  5. Educate patients on their care plans, including medication management, appointment scheduling, and other necessary resources.
  6. Advocate for patients and advocate for their needs within the healthcare system.
  7. Conduct comprehensive assessments of patient needs, including physical, emotional, and social factors.
  8. Ensure accurate and timely documentation of patient interactions and care plans.

Job Qualifications
  • Certified Medical Assistant: The Ideal Candidate Should Hold A Current Certification As A Medical Assistant From An Accredited Institution. This Ensures That They Have The Necessary Knowledge And Skills To Perform Clinical Tasks And Provide Quality Patient Care.

  • Experience In A Primary Care Setting: Candidates Should Have At Least 1-2 Years Of Experience Working In A Primary Care Setting. This Will Demonstrate Their Understanding Of The Role Of A Patient-Centered Medical Home And Their Ability To Work Effectively With A Team Of Healthcare Professionals.

  • Strong Communication Skills: As A Care Coordinator, The Candidate Will Be Responsible For Communicating With Patients, Healthcare Providers, And Other Stakeholders. Therefore, Excellent Verbal And Written Communication Skills Are Essential For This Role.

  • Knowledge Of Electronic Health Records: Musc Health Uses An Electronic Health Record System To Manage Patient Data. The Ideal Candidate Should Have Experience Working With Ehrs And Be Comfortable Navigating And Utilizing This Technology.
