  1. Provide case management services to pediatric patients and their families to ensure they receive appropriate and high-quality healthcare.
  2. Collaborate with the interdisciplinary team to develop and implement individualized care plans for each patient.
  3. Conduct thorough assessments of patients’ medical, emotional, and social needs to determine appropriate interventions and resources.
  4. Serve as a liaison between patients, families, healthcare providers, and insurance companies to coordinate and facilitate care.
  5. Educate patients and families about their medical conditions, treatment options, and available resources to promote self-management and improve health outcomes.
  6. Monitor patients’ progress and make necessary adjustments to their care plans to ensure continuity of care.
  7. Advocate for patients’ needs and rights, and ensure they receive culturally sensitive and patient-centered care.
  8. Maintain accurate and timely documentation of patient interactions, care plans, and outcomes.
  9. Collaborate with community resources and organizations to provide patients with access to necessary services and support.
  10. Participate in performance improvement initiatives to enhance the quality and effectiveness of case management services.
  11. Stay updated on current healthcare trends, policies, and regulations related to pediatric care and case management.
  12. Serve as a mentor and resource for other team members, providing guidance and support in complex cases.
  13. Demonstrate a commitment to continuous learning and professional development to maintain and enhance knowledge and skills in pediatric case management.

Job Qualifications
  • Bachelor's Degree In Nursing From An Accredited Institution

  • Current And Unrestricted Rn License In The State Of South Carolina

  • Minimum Of 2 Years Of Experience In Pediatric Nursing Or Case Management

  • Knowledge Of Healthcare Systems, Regulations, And Reimbursement Processes

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And Build Rapport With Patients And Families.
